Our street outreach team plays a critical role in identifying individuals who are experiencing houselessness and connecting them with programs to better address their needs.

Program Details

Identify individuals who are experiencing houselessness.
Help individuals find safe and secure shelter based on their needs. This can start with congregate shelters but the goal is to find permanent housing.
Assist in obtaining supplies, such as sleeping bags, tents, food, masks, vaccines, naloxone, clothing and food.
Go to where you are!

Community Connect - HMIS

Remember, if you are experiencing houselessness, you should contact Community Connect at 503-640-3263 on a monthly basis to confirm housing status. This ensures that you stay active in the HMIS system so that you can receive services when they become available. If you do not, you run the risk of being kicked out of the system and may have to restart the process.

Housing First Approach

Our team is a big believer in shelter first. For immediate needs, we have two low-barrier shelters, one in Tigard and one in Beaverton. However, the goal is to find individuals or families, long-term housing. Our team is experts in providing resources and pointing you in the right direction.
